Step 1: Complete a Parent Financial Statement (PFS).
Step 2: Upload all required forms by February 1 to SSS or mail to SSS at P.O. Box 449, Randolph, MA 02368-0449:
- W-2 Forms from Current Employer for three years (including the current year)
- IRS Form 1040 with all schedules for three years (including the current year)
- IRS Form 4506-T (signed, but not dated)
- Any IRS forms that are in the student's name (1040, 1099, etc.)
- Business and Farm Statement (for all business taxes related to the family)

Financial aid decisions and other related information will only be discussed with financial aid or admissions personnel.
